Add message to appointment confirmations

Right now we can send out appointment confirmation at the time of booking that include time/date. Can we add the option to add clinic address/instructions.

Also, it would be great to tell the patients whether this was an office appointment or telephone appointment. At present, Notify just says You have an appointment; I have had a lot of patients show up to the office when it was a telephone appointment.
