Consultations - list different services each referral has been faxed to

Clicking Consultations header to bring up a list against each referral letter, of each service that letter has been faxed to (eg failed/rejected fax to one gastro, re-point the same referral letter to a different gastro and display both of these times/dates in the history. Same if it needs to be re-pointed to a 3rd or 4th specialist.
Currently Avaros just overwrites the original referral and only lists the latest service it has been faxed to. To maintain history, an entirely new Consultation referral needs to be made
