I second this! It would be preferred to have contact information, as currently it says to call clinic to cancel but there is no reminder of clinic address or number. We see patients by referral, and so they have not been the one to initiate the call/ booking process, and might not have the phone # handy to be able to call us to cancel or reschedule.

It would be even better to allow for customization to allow for including clinic website, instructions for prep etc.
